Providence Health & Services Supervisor Care Management Services in Anchorage, Alaska
Providence is calling a Supervisor Care Management Services (Non RN-Oncology) (Oncology Support Services, Day Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will have the following responsibilities:
Work closely with all patient care units/departments/services and interdisciplinary team members to resolve issues related to care management
Supervise the Care Management services to accomplish the goals of the program
Ensure regulatory requirements and accreditation standards are applied to all activities and reporting
Assist Senior Manager Care Management Services in developing department quality and performance targets and analyzes actual data against them; share responsibility with manager for specified month-end reports; collaborates in development of formats for standard and specialized reports; assists with collection and analysis of data
Manage own patient assignment/caseload to ensure adequate coverage of care management services across the department
Educate care management caregivers and interdisciplinary team on care management standards of practice, collaborates with providers on alternatives to care based on patient's insurance coverage and preferences; participates as a member of project teams and committees related to care management/care coordination/and oncology
Work with local, statewide, and national community stakeholders, the Cancer Committee, and other cancer care advocates on projects, grants, development of survivorship programming for the Cancer Center
Required qualifications for this position includes:
Master's Degree in Social Work
Unencumbered Alaska LCSW license within twelve (12) months of hire
Oncology Social Work Certification within three (3) years
Preferred qualifications for this position includes:
Three (3) years of Oncology Social Work Certification (OSW)
Two (2) years of Management experience supervising Social Workers
