Social Worker MSW - Home Health
Schedule: Flexible weekdays, Day shift (20 hours/week)
Job Description:
Provides professional social work services to improve or maintain the social, emotional, functional and physical health status of the client and support systems. Works in collaboration with client, family, physician, other staff and community agencies to assess, plan, implement and evaluate appropriate treatment. Collaborates with interdisciplinary team in the process of assessment, evaluation and coordination of services for client and/or family. Assists to identify, interpret and reduce psychosocial factors contributing to illness. Interviews, coordinates and plans services to meet the social and emotional needs of clients and/or family. Provides case management and assists with referral process. Provides social work consultation to agency.
Required qualifications:
Master's Degree in Social Work
Upon hire: Alaska Masters Social Worker License Or
Upon hire: Alaska Clinical Social Worker License
Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information
2 years experience in acute care medial social work
Must have experience with patient and family assessment and diverse populations to include the chronically ill, older adult and underserved populations

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
