Providence Health & Services Social Work LCSW in Anchorage, Alaska

Description:

Providence is calling a Social Work LCSW (Utilization-Case Mgmt, Day Shift, 1.0 FTE) at St Elias Specialty Hospital in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position, you will:

  • Assess the functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network

  • Formulate a plan of intervention acceptable to the patient, family and medical team; proactively facilitates adjustment in the care plan or services when necessary to promote enhanced outcomes

  • Document all interventions in the patient medial record both timely and accurately including all elements of the discharge plan

  • Provide advocacy, assistance, support, counseling and crisis intervention to patients and families. In the case of suspected abuse/neglect/exploitation, the Social Worker documents and reports where required. Assist with hospital-sponsored groups

  • Function in a liaison role between the hospital and community in making community resources available to the patient and family

  • Collaborate with all members of the healthcare team to develop, manage and communicate discharge plans. Maintain current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term and low-income patients

  • Provide professional education to staff

Qualifications:

Required qualifications for this position include:

  • Masters Degree in Social Work

  • Licensed by the State of Alaska as a Social Worker

  • Basic Life Support (BLS) certification is required, new hires have thirty (30) days from the date of hire to obtain BLS cards if not current at the time of hire

  • 3 years acute care hospital experience

  • Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services

About St. Elias Specialty Hospital

St. Elias Specialty Hospital is owned by Providence Health System. The hospital has 59 beds, almost all private rooms. The facility features state of the art equipment to provide extended inpatient acute care for patients.

At St. Elias Specialty Hospital, we provide customized, physician-driven services for patients requiring longer stays in an acute care environment due to multiple or complex medical conditions. We are not a permanent placement for patients; rather we are an answer for many patients who are acutely ill and need to take the first steps towards rehabilitation or home.

Most of our patients are admitted from a traditional acute care hospital, often from the Intensive Care Unit. Families of patients are welcome to contact us to see if we might be an option for their loved one.
