In this position you will:
Perform functions within the scope of practice of a nurse with specialized training in wound and foot care including assessment, care planning, implementing and evaluating treatment plans. Perform risk assessment screening for skin integrity, peripheral neuropathy, and vascular problems.
Document care and treatment according to professional practice standards.
Work effectively with the Interdisciplinary and Direct Care Teams to effectively prevent, identify, assess, treat, monitor and evaluate wounds as well as special needs related to foot and nail care.
Act as a clinical resource and wound/foot care expert. Remain current in professional practice standards/national pressure ulcer prevention strategies. Role model collaboration and partnership with physicians, staff, residents, family and other departments.
Coordinate the functions of the wound care team and wound care rounds.
Evaluate skin/wound care products and works with materials department to maintain inventories of the most effective, appropriate products at reasonable costs.
Participate in the orientation and training of new staff. Remains current on best practices related to wound care, foot care and preventative measures.
Collect data to monitor the incidence, characteristics and status of wounds within the facilities and identifies quality indicators and facilitates individual and facility-wide initiatives to improve outcomes.
Analyze data to identify and report trends, recommend/initiate quality improvement initiatives, implement actions and evaluate outcomes.
Prepare reports as needed to support systematic and ongoing quality improvement processes.
Provide Patient-Resident-Family education to promote wellness.
Contact other members of the healthcare team as needed. Completes physician orders, ensuring diagnosis or condition with each order. Evaluates justification for each medication as medically necessary. Collaborate with all other healthcare professionals as needed. Observe and report significant findings to other healthcare professionals.
Initiate plan of care specific to care of ostomy, draining wound, /fistula, incontinence, or pressure sores/dermal wounds.
Required qualifications for this position include:
Bachelor’s degree in Nursing
Current Alaska RN license
Must obtain Wound Care Certification within 6 months and Foot Care Certification within 18 months of hire
Current Basic Life Support (BLS) card must be from American Heart Association (AHA) upon hire
2 years in a health care facility (acute care, long term care, or rehabilitation center
- Wound care experience
