Providence is calling a RN Patient Flow Coordinator (Patient Care Res Svcs, Day Shift, 1.0 FTE, 40 hours per week) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will have the following responsibilities:
In real time, expedite, prioritize, synchronize and mediate clinical issues, remove barriers to discharge and address delays in care
Triage alerts and situational awareness via investigation per the electronic medical record and additional clinical resources
Manage deviations from patient care pathways; resolve bottlenecks, variation in service level agreements, optimize communication and prioritization with support services and a variety of clinical departments
Provide leadership and work with SOC colleagues to address patient flow issues
Work collaboratively with all members of the Discharge Team
Collaborate with SOC operations and all other pertinent departments to identify process improvement strategies, reduce patient length of stay (LOS), trend delays, support quality initiatives and enhance patient safety
Assist with feedback mechanisms to inform operational team members, mitigate over capacity situations, and manage surge with patient volumes
Required qualifications for this position include:
Bachelor's Degree in Nursing
Registered Nurse License with the State of Alaska
3 years clinical experience with two years of progressively responsible management or supervisory experience in acute-care facility
Preferred qualifications for this position include:
- Master's Degree in Nursing
