Providence Health & Services RN Case Manager in Anchorage, Alaska
Providence is calling an Registered Nurse (RN) Case Manager (Utilization-Case Mgmt, Day Shift, 1.0 FTE) to St Elias Specialty Hospital in Anchorage, Alaska. In this position you will:
Uses a systematic process and clinical skills to assess, plan, implement and evaluate an individual plan of care for a patient that spans the continuum of care. The plan of care is developed in conjunction with the attending Provider, the patients and the patients family or caregiver(s).
Monitors and reassesses the plan of care to ensure that quality, quantity and goal achievement are appropriate and cost-effective.
Maintains, updates and improves clinical expertise and care management skills.
Provides appropriate education for patient, family and staff regarding community support systems.
Demonstrates the ability to utilize regional population risk stratification information and processes to identify appropriate members for enrollment into case management for complex longitudinal care in collaboration with clinicians and other members of the health care team.
Develops (with patients and family members) and documents a patient-centered care plan that addresses patient centered goals that are specific, attainable and measurable.
Documents ongoing follow-up and monitoring of effectiveness of the interventions and reinforce the treatment plan.
Participates in the education of patients/family members/staff and clinicians.
Contributes to ongoing process improvement including related procedures, policies, patient support and documentation tools.
Required qualifications for this position include:
Registered Nursing Degree/Diploma
All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree or higher within 3 years of hire
Licensed as Registered Nurse (RN) in the State of Alaska upon hire
2 years of clinical nursing experience
Preferred qualifications for this position include:
3 years of clinical nursing experience
1 year of Utilization Management-review and Discharge-planning experience
About St. Elias Specialty Hospital
St. Elias Specialty Hospital is owned by Providence Health System. The hospital has 59 beds, almost all private rooms. The facility features state of the art equipment to provide extended inpatient acute care for patients.
At St. Elias Specialty Hospital, we provide customized, physician-driven services for patients requiring longer stays in an acute care environment due to multiple or complex medical conditions. We are not a permanent placement for patients; rather we are an answer for many patients who are acutely ill and need to take the first steps towards rehabilitation or home.
