Providence Pharmacist Residency (PGYI) in Anchorage, Alaska
Providence is calling a Pharmacist Resident (Pharmacist Residency Prog, Variable, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will:
Review and interpret physicians orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, condition and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary.
Review patient records and orders regarding drug therapy, recommends and initiates changes that are consistent with the patient's age; is successful in improving patient outcome through monitoring and intervention consistently.
As required, regularly consult with physicians and nursing staff members concerning the use, availability, dosage form, side effects and possible incompatibilities of new as well as currently used drugs; always takes time to clearly explain information to concerned patients.
Follow proper computer order entry procedures.
Mix IV additive solutions and chemotherapy as required using good aseptic technique within the laminar flow hood.
Be responsible for the completion of all assigned work procedures during the shift while maintaining the highest standards of quality.
For medications defined as part of PHS's clinical services (DUE, pharmacokinetics etc.), concurrent interventions and follow-up will be made in order to assure the appropriateness, safety, and efficacy of the medication.
Required qualifications for this position include:
Graduate from an accredited college or university of pharmacy
If not licensed in the State of Alaska at initiation of the program, the resident should have initiated licensure application and obtain licensure from the Alaska State Board of Pharmacy during the residency
Preferred qualifications for this position include:
Previous experience in hospital pharmacy practice as a student, intern or pharmacist
