Providence Health & Services Patient Navigator (MSW) - Oncology Support Services in Anchorage, Alaska
Providence is calling a Patient Navigator (Oncology Support Services, Day Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Perform a comprehensive assessment.
Assist the patient, their families, and/or caregiver in establishing goals intended to reduce or eliminate barriers to care, provide education, and assistance in coping with cancer related challenges.
Re-assess patient's progress and needs and recommends necessary change to accomplish goals.
Evaluates patients and their related support systems with a complete biopsychosocial and spiritual assessment. Uses social work theory principals and methods to formulate and implement a psychosocial plan of action based on the patient needs and strengths.
Confers with team members and/or necessary providers regarding patient's barriers, progress, and plans, and provides documentation of the same.
Provides accurate and timely documentation.
Serves as a resource for other cancer healthcare providers, community resource agencies, and other groups in the local and statewide healthcare community. Coordinates and collaborates services for cancer patients and their families or significant others. Provides community and staff education as requested and supported by leadership.
Provides case management to assure appropriate referral to in-house and community resources with continuing evaluation of the services being provided.
Required qualifications for this position include:
Master's degree in Social Work (MSW)
4 years of experience in a medical or counseling setting
Ability to communicate well and interact with multiple health care professionals, including medical and allied staff
Preferred qualifications for this position include:
Licensed Master of Social Work (LMSW) or related field; Licensed Clinical Social Worker (LCSW) preferred
Knowledge of Alaska community resources
