Providence Health & Services Medical Social Worker (MSW) - Primary Care in Anchorage, Alaska

Description:

Providence is calling a Medical Social Worker (MSW) (Pmg Senior Care, Day Shift, 1.0 FTE) to Providence Physician Service Organization in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will:

  • Exemplify excellent customer relations with clients, visitors, and other employees; shows courtesy, friendliness, and respect

  • Make referrals, based on identified needs, to appropriate agencies/services or provide information and assists clients for self-referral

  • Provide advocacy as needed to encourage participation from clients in resolving problems (such as transportation or language barriers)

  • Maintain files that document services in accordance with clinic policy

  • Assist client in accomplishing tasks necessary to access services such as filling out appropriate forms, obtaining necessary documentation or authorizations

  • Maintain professional standards and follow the treatment policies and procedures of the clinic, as applicable federal, state and local regulations.

  • Completes a daily schedule which documents every client visit, and time of visit according to clinic policy

  • Report any unusual incidents to their supervisor. The Social Worker is a state mandated reporter, he/she will report the state any suspected case of child/elder abuse or neglect as mandated by DHSS

  • Communicate clearly, both orally and in writing, with clients, providers, community professionals, and interdisciplinary team members. This includes maintaining records that are clear and concise regarding psychosocial and/or needs assessment, advocacy, referrals, and follow-up care. The documentation must reflect legal mandates for reporting abuse/neglect

  • Develop working relations with both internal and external service system networks; networks with resource providers serving this population to develop strategies to improve services delivery

  • Demonstrate knowledge and skills necessary to provide care appropriate to the age and/or diversity of the patients served

  • Able to identify and categorize each patient’s age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients

  • Facilitate teaching and supervision of graduate social work interns

  • Contribute to the plan of care as established by the inter-disciplinary team and work to obtain the necessary services and resources related to the patient’s needs

  • Assist with grant writing and program development in case management for the Senior Clinic

  • STANDARDS OF CARE:

  • Actively support and incorporate the mission and core values into daily practice, as well as customer service standards as detailed by PAMC administration. Treat all others with respect and demonstrates excellence, justice, stewardship and compassion in daily work relationships

  • Maintain confidentiality of all information related to patients, medical staff, employees and as appropriate, other information

  • Demonstrate positive and professional interpersonal relations in dealing with others, recognizing cultural diversity, including patients and families, employees, management team, medical staff, volunteers and community members, so that productivity and positive relations are maximized. Maintain appropriate boundaries in all interactions

  • Understand and is compliant with hospital-wide and department policy related to Medical Social Work. Also comply with hospital standards to assure regulatory requirements are met such as but are not limited to: JCAHO, OSHA, HIPPA, State and Federal requirements

  • Complete initial and annual Competency Plan for assigned job and department

Qualifications:

Required qualifications for this position include:

  • Master's degree in Social Work (MSW)

  • Alaska Licensed Masters of Social Work (LMSW) or Alaska Licensed Clinical Social Worker (LCSW)

Preferred qualifications for this position include:

  • 2 years of acute care Medical Social Worker experience

  • Care Coordination license
